Anyone under the age of 18 who wishes to begin employment must undergo a medical examination (juvenile occupational safety examination). Employers are not permitted to hire minors without this examination.

The following are required for this:

• an examination authorization form (serves as a billing document and must be submitted to the examining physician) and
• a questionnaire (used to describe your current physical and mental health status and must be submitted, fully completed, to the examining physician).
